How does the Care Assessment Tool work?
Our online Care Assessment Tool will guide you through six short questions to help determine the level of dependency you or your loved one requires. Once completed, you will be provided with some useful next steps in exploring care options.
The questions you will be asked cover:
Physical Function
How well can you / your loved one manage physical activities such as moving around the house and walking up and down the stairs?
Mental Function
How good is your / your loved ones’ memory, orientation and communication as well as behaviour, cooperation and acceptance of support?
Personal Care & Outlook
How well are you / your loved one able to manage personal care, hygiene and medication, as well as your level of social activity?
